Mucus biology leap is a path to airway clearing

People with chronic respiratory diseases such as asthma, cystic fibrosis and chronic obstructive pulmonary disease (COPD) typically produce an excessive amount of thick mucus in the lungs. This obstructs the airways, making it difficult to breathe.

A Melbourne research team led by from the has discovered new insights into the molecular mechanisms driving mucus viscosity. It is hoped that understanding how mucus thickness is regulated could help to improve airway-clearing treatment options for patients with respiratory conditions.

Mucus mainly consists of water and mucin glycoproteins, which are very long protein strands coated with glycans 鈥 a type of sugar molecule.

Associate Professor Goddard-Borger said the study findings 鈥 published in the journal 鈥 revealed that proteins called 鈥榯refoil factors鈥 interact with mucins by recognising and binding to the unique glycan signatures on their surface.

鈥淭refoil factors have long been known to make mucus more viscous (thicker), and it has been postulated that this thickening occurs in respiratory diseases. However, until now we did not completely understand how the trefoil factor proteins achieved this.鈥

Understanding what trefoil factors bind to and how they do this represents a significant leap forward in understanding mucus and how it functions in the respiratory, gastrointestinal and reproductive tracts. Associate Professor Goddard-Borger said the research showed trefoil factors had two glycan-binding sites and could cross-link mucins strands to make the mucus gel more rigid.

鈥淲ithin mucus, trefoil factors essentially 鈥榮taple鈥 the mucin strands into a mesh: the more staples, the denser the mesh and the thicker the mucus becomes.鈥

Improving therapies for blocked airways

Associate Professor Goddard-Borger said that ongoing research aims to inhibit the bonds created between trefoil factors and mucin strands, and that the development of such a technology could lead to new therapeutics for the treatment of respiratory diseases.

鈥淎 healthy amount of mucus is very important for capturing and clearing potential threats to the lung, such as dust particles, dead cells and bacteria, so we鈥檙e not looking to remove mucus altogether.

鈥淲e are seeking to develop innovative approaches for reducing the viscosity of the mucus to aid in clearing excess mucus from the lungs of patients with chronic respiratory disease.

鈥淭he next step is to work with commercial collaborators to progress our vision to develop new mucolytic drugs that can more effectively clear mucus from the airways. Achieving this could make a significant impact on the quality of life and life expectancy of people struggling with debilitating respiratory conditions,鈥 Associate Professor Goddard-Borger said.
