After a second embryo incident on 5 June, this time聽at its Clayton laboratory in Victoria, has extended the scope of the independent review being conducted by Fiona McLeod AO SC 鈥� the review that is in relation to a similar Brisbane incident (as announced on 10 April 2025).

In the latest, 5 June incident, a patient鈥檚 own embryo was incorrectly transferred to that patient, contrary to the treatment plan which designated the transfer of an embryo of the patient鈥檚 partner. 鈥淢onash IVF has extended its sincere apologies to the affected couple, and we continue to support them,鈥� the company said.

鈥淐ommencing immediately, Monash IVF will implement interim additional verification processes and patient confirmation safeguards over and above normal practice and electronic witness systems, to ensure patients and clinicians have every confidence in its processes.

鈥淲hilst industry leading electronic witness systems have and are being rolled out across Monash IVF, there remains instances and circumstances whereby manual witnessing is required.鈥� Monash IVF said it has disclosed the incident to the relevant assisted reproductive technology regulators 鈥� the Reproductive Technology Accreditation Committee certifying body and Victorian Health Regulator.

Monash IVF also said it has notified its insurers. The company said it expects the incident to fall within the scope of its insurance coverage. Monash IVF said it will provide an update on the findings of the expanded review in due course, with an internal investigation into the Clayton incident also being conducted.
