Consultmed, Personify Care partner to deliver end-to-end patient pathway

and have partnered to deliver an end-to-end patient pathway from the first GP visit and referral to specialist services, triage, admission, consent and enrolment into a digital patient pathway.

This collaborative approach and integration will be launched later this year at Wandi Nerida, a specialist residential eating disorder recovery centre in Queensland and will be made available to other health providers managing elective surgery and outpatient services.

Consultmed CEO A/Professor Vikram Palit said, 鈥淪o often we expect patients to do the heavy lifting, carry around their paper documents, find the right specialist, book themselves in and, in some cases, work out how to admit themselves to hospital.

鈥淲e are looking to address these gaps and the fragmentation that exists in the system by connecting Consultmed鈥檚 digital referral platform with Personify Care鈥檚 digital patient pathways. This means that at every stage, from going to your GP, referral, triage, consent, pre-admission and discharge, the process is seamless, and we prevent unnecessary duplication and variation in care."
